Ability to work independently and in a team environment. Ability to work effectively and build relationships with various levels throughout the company. Responsibilities NICHE (Nurses Improving Care for Healthsystem Elders) is a program of the Hartford Institute for Geriatric Nursing at New York University College of Nursing, The vision of NICHE is for all patients 65 and over to be given sensitive and exemplary care. The mission of NICHE is to provide principles and tools to stimulate a change in the culture of healthcare facilities to achieve patient-centered care for older adults. NICHE is currently seeking two Geriatric Advance Practice Nurses for the following positions: GAPN, Rural E-APN Project A full-time Geriatric Advanced Practice Nurse (GAPN - CNS and/or NP) position with the Nurses Improving Care for Healthsystem Elders (NICHE) program. The GAPN will work from a home office to put into practice the on-line mentoring role essential to a new, innovative, "e-GAPN" model of NICHE implementation. The GAPN will facilitate NICHE implementation in 20 rural or small hospitals in upstate New York and promote a rural-urban collaboration with established NICHE sites in Rochester and Syracuse. Responsibilities With a direct report to executive project management, the Project Manager will lead and coordinate research, data, and administrative activities for the New York State Medical Liability Reform and Patient Safety Model (“the NYS Model”). The NYS Model puts patient safety first and strives to reduce preventable injuries, improve doctor/patient communication, and ensure that patients receive fair and quick compensation for medical injuries, while also reducing the incidence of frivolous lawsuits and cost of insurance premiums. It is a comprehensive pilot that reforms the medical liability system. The model incorporates evidence-based interventions, including a Disclosure and Early Settlement program and a culture of patient safety at five large academic medical centers in New York City.

Responsibilities include: Manage, direct, implement and coordinate project activities and oversee work plan compliance. Serve as the primary contact for the project partners. Oversee and collaborate with the project partners to manage and support the implementation and analysis of the Disclosure and Early Settlement Program. Oversee and collaborate with the project partners to assess the program components, identify best practices, design the research model and develop options to manage and support the implementation and analysis of the Patient Safety Demonstration Program. Oversee administration and analysis of patient safety surveys. Work with the project partners to anticipate problems and to identify and tailor solutions to challenges arising from the implementation of the projects. Facilitate and support data sampling, collection, and evaluation for both programs. Develop and provide technical program research and analysis assistance to the project partners regarding planning, project requirements, reporting standards and alternate methods, and documentation. Monitor the work of the project partners to ensure deliverables are being met and the project is implemented according to schedule. Ensure compliance with time and effort reporting, contract vouchering, and any other necessary documentation or reporting. Supervise the Patient Safety Nurse in support of both programs. Supervise an Analyst in the collection, interpretation and evaluation of data for both programs. Assure timely production by all project participants with project deliverables. Other related duties as assigned. Frequent travel to New York City to meet with project partners to oversee and implement above functions.
Qualifications Required Bachelor’s Degree. Five years of professional experience in hospital risk management, nursing, public health, public administration, public policy, management or a related field, including three years of experience supervising staff.
Qualififcations Preferred Masters Degree in Nursing, Public Health, Public Administration, Management, Risk Management, Law or a related field. Experience implementing relevant programs. Strong writing, verbal communication, interpersonal and organizational skills. Strong project management skills.

Qualifications Required Bachelors degree. License and current registration as a professional registered nurse in NYS. Four years of experience providing nursing services. Experience in planning, evaluating, or administering health services or teaching in a health related program. Completion of a Masters degree in nursing or a health related field may substitute for one year of the required experience.
Qualififcations Preferred Registered nurse with a minimum of one year experience in nurse management. Background in training and education of health care providers, Excellent organization skills and communication skills, which includes both public speaking and the preparation of written materials including coordinating training events, and preparing and delivering educational materials. Masters Degree in Nursing, Public Health, Public Administration, Management, Risk Management or a related field. Experience in implementing relevant programs.

Responsibilities Primary Care Health Information Consortium (PCHIC) was established to assist community health centers in New York City with the implementation of health information technology, a powerful tool for improving the quality, efficiency and effectiveness of patient care. In this highly visible full time grant funded role, you will oversee the staffing of the PCHIC Clinical Committee and provide day-to-day coordination of projects relating to the use of Health Information Technology to improve clinical care in community health centers. In addition, you will manage the scope, budget, and schedule of these projects, and assist the PCHIC Director with other work as needed. Qualifications Required Successful candidate will be a licensed healthcare professional with 2+ years of patient care experience, preferably in a community health center, along with at least 2 years of project management experience, ideally related to health information technology. Ability to work with minimal supervision, solid budget development and management experience, and excellent oral/written communication skills will be expected, as will strong presentation and analytical skills. PC proficiency with MS Office applications (Word, Excel, PowerPoint, Outlook) also essential.
